Home and Community Based Services Case Manager

The Northwest Georgia Regional Commission Area Agency on Aging is seeking a qualified candidate for the Home and Community Based Services (HCBS) Case Manager position.  The HCBS Case Manager is responsible for case management implementation for clients on non-Medicaid waiting lists.

This position requires frequent travel and candidate must possess a degree in Social Work, Nursing, or a Bachelor’s degree in a related field and prior experience in case management.
